Geintegreerde geneeskunde: aandacht, empathie, tijd en expertise!

De termen die gehanteerd worden om vernieuwing binnen de geneeskunde aan te geven zijn nogal verwarrend.  Moet het nu integratieve geneeskunde of geintegreerde geneeskunde worden?

Integratieve geneeskunde is niet zoals velen denken die vorm van geneeskunde die het beste uit zowel de reguliere als complementaire behandelsvormen combineert. De term integratieve geneeskunde omvat namelijk veel meer. De Amerikanen omschrjiven het als:

"Integrative Medicine is the practice of medicine that reaffirms the importance of the relationship between practitioner and patient, focusses on the whole person, is informed by evidence, and makes use of all appropriate therapeutic approaches… to achieve optimal health and healing"

In die definitie zien we aspecten optreden die bekend zijn uit de definitie die de WHO gaf van wat gezondheid is. Integratieve geneeskunde heeft dus enkele duidelijk uitgangspunten:

  1. de relatie tussen behandelaar en patient staat centraal (empathie, tijd, aandacht en expertise),
  2. focus ligt bij de patient als geheel en niet bij de ziekte of de symptomen, 
  3. de behandelingen heben een basis in de EVM,
  4. de behandeling selecteert het beste uit alle therapeutische mogelijkheden (complementair en regulier)
  5. opdat optimale gezondheid en heling (sic!) mogelijk wordt.

Integrale geneeskunde

Daarnaast is de term integrale geneeskunde al langer bekend in Nederland, een term die in de jaren 70 geintroduceerd werd om aan te geven dat geneeskunde  zich richt op de gehele patient. Omdat dat aspect van "het geheel" wat moeilijk is – ook de huisartsgeneeskunde richt zich immers op de gehele patient – en de integrale geneeslkunde van de jaren 70 vooral in contrast met de reguliere geneeskunde gedefinieerd werd, is deze term eigenlijk obsoleet.

Geintegreerde Geneeskunde

De term geintegreerde geneeskunde lijkt vooralsnog de beste vertaling van de Amerikaanse stroming die zich aanduidt met de term Integrative Medicine. De Engelsen gebruiken wel de term Integrated Medicine.

In het BMJ (2001) staat de volgende omschrijving:

Integrated medicine is not simply a synonym for complementary medicine. Complementary medicine refers to treatments that may be used as adjuncts to conventional treatment and are not usually taught in medical schools. Integrated medicine has a larger meaning and mission, its focus being on health and healing rather than disease and treatment. It views patients as whole people with minds and spirits as well as bodies and includes these dimensions into diagnosis and treatment. It also involves patients and doctors working to maintain health by paying attention to lifestyle factors such as diet, exercise, quality of rest and sleep, and the nature of relationships.

    Het lijkt me dat jullie een goed pleidooi hebben voor de term Geintegreerde Geneeskunde. Duidelijk dat het in ieder geval meer is dan een fusion van wat verschillende behandelingen!
