Doorbraak voor revalidatie in 2011: oude mensen lopen beter na acupunctuur

Het klinkt te mooi om waar te zijn. Duitse geriaters van een universiteitsziekenhuis toonden in een placebo-gecontroleerde studie aan dat echte acupunctuur op een acupunctuurpunt op het been de revalidatie van bejaarden significant beter maakt. Het lopen gaat met echte acupunctuur duidelijk en meetbaar beter dan bij het inzetten van een nep acupunctuurpunt.  Een doorbraakstudie! Hiermee is wederom bewezen dat acupunctuurpunten een specifieke werking hebben, en dat dat meer is dan zo maar wat in het wilde weg prikken! Dit alles in het Januari-nummer van het hoog aangeschreven medische tijdschrift voor revalidatieartsen, 'Arch Phys Med Rehabil' (Vol 92, January 2011)

Methodologisch was de studie zeer strak, zoals dat van Duitse wetenschappers ook kan worden verwacht:[1]

In een ‘Multiple-blinded, randomized, controlled intervention trial‘ werden 60 geriatrische patienten gevolgd tijdens hun revalidatie.

Beiden groepen (31 en 29 patienten) kregen acupunctuur, de ene helft zinloze nep acupunctuur en de andere helft zinvolle acupunctuur op een bestaand acupunt.

De puntkeuze was alsvolgt:

For verum treatment, the acupuncture point stomach 34  was used, located 2 cun proximal the superior lateral border of the patella. This acupoint was chosen according to the theory of Chinese Med- icine as described by the Heidelberg Model of TCM.41 This point, besides its analgetic effect, has traditionally been used for weakness of the legs and neurologic deficits in the lower extremities as induced by palsy or hemiplegia. Control treat- ment was performed at a nonspecific skin point with no direct relation to any conduit (meridian) between the felleal conduit (gallbladder meridian) and the lateral posterior border of the femur and at the same height as the verum point 

Indrukwekkende resultaten! 

De resultaten waren indrukwekkend:

"All gait parameters showed statistically significant improvement after verum treatment compared with control treatment (velocity, cadence, stride length, cycle time, step time, single support, double support: P values all 0<.05) except for the base of support (P=0.163). Effect sizes achieved by 1-time stimulation of an acupoint were low and ranged from .08 to .24. No severe adverse clinical events related to the intervention occurred."

Omdat even te helpen met het lezen van de tabel: terwijl bijvoorbeeld de mensen die alleen een nepnaald kregen qua loopsnelheid van 87.9 naar 86.9 gingen (langzamer liepen dus), liepen de met echte acupunctuur behandelde mensen van 79.57 naar 85.19, dat is dus 6 units sneller!

Ook de loopkadans nam 5 units toe bij de met echte acupunctuur behandelde bejaarden, terwijl de nep acupunctuur geen enkel effect op de kadans had. En zo zie je bij vrijwel elke gemeten parameter grote verbeteringen bij de acupunctuur patienten en geen verbeteringen bij de placebo-behandelingen. 

De auteurs begonnen met in de inleiding te stellen dat het voor bejaarden enorm belangrijk is om een goede mobiliteit te blijven houden, en dat loopstoornissen een grote risico factor zijn voor vallen, fracturen en overlijden. 

ADEQUATE GAIT MOBILITY is an important quality of life issue in older patients and important to preserve functional independence. Gait disorders represent high-impact risk factors for falls/fall injuries, and severe mobility limitation, hospitalization, and mortality in older people. 

Goede revalidatie van bejaarden is dus zeer belangrijk. Vandaar dat de geriaters verder keken dan hun neus lang was en de waarde van acupunctuur aldus in kaart gebracht hebben:

Alternative therapeutic approaches such as acupuncture after TCM principles may hold options for those who cannot follow strenuous training regimens or may fortify benefits already achieved by established rehabilitative programs. However, despite the widespread use of acupuncture in recent years, its efficacy remains controversial in different areas of application.  

Hun uitgangspunt bij deze studie was om het effect te meten van het eenmalige prikken van een acupunctuurpunt bij bejaarde, kwetsbare en zieke ouderen tijdens een revalidatie-proces :

Therefore, the objective of the present study was to observe the acute effects of a single acupoint stimulation based on TCM criteria in frail, multimorbid patients during geriatric rehabilitation. A rigorous multiple blinding design and objective technical measurements were used to ensure that any effects of intervention were free of bias of patients, therapists, or assessors. Based on the study hypothesis, we expected significant effects on different parameters of gait pe
rformances only in the group undergoing the real acupoint stimulation as defined by TCM criteria (verum) in comparison to nonacupoint skin stimulation (control). 

De conclusie van de geriaters was dat acupunctuur niet alleen significant een loopverbetering gaf, maar dat het effect ook klinisch relevant was:

This is the first double-blind study to show statistically significant and potentially clinically relevant effects of needle acupuncture on gait performance in unselected geriatric patients on a rehabilitation ward.

It will be a worthwhile research goal to relate clinical research to applied TCM practice to unveil underlying mechanisms and optimize intervention strategies by a prolonged time period of application and modified frequency, duration, and quality of stimulation or acupuncture based on individual TCM diagnosis for different indications.

Eenmalige acupunctuur op een beenpunt blijkt dus de revalidatie van bejaarde en zieke patienten meetbaar te verbeteren, zonder relevante bijwerkingen; een goedkope en zinvolle interventie dus bij de revalidatie van deze patienten.
