Hypnotherapie: 2009, jaar van doorbraak

Hypnose of hypnotherapie heeft in het jaar 2009 de overgang gemaakt van 'alternatief' naar 'regulier'. Allereerst promoveerde in Nederland de kinderarts Dr Ariane Vlieger op een mooi proefschrift over hypnose, aan de Universiteit van Amsterdam. In haar proefschrift aandacht voor de waarde van hypnose bij onbegrepen buikklachten bij kinderen. Het resultaat van dit proefschrift is dat anno 2009 meer reguliere kinderartsen kinderen met onbegrepen buikklachten gaan verwijzen naar gecertificeerde hypnotherapeuten. Hier sommen we enkele onderzoeksrichtingen op, die tot resultaat hadden publicaties over de zin en de effecten van hypnotherapie in het jaar 2009.

In 2009 ook een groot aantal publicaties op het gebied van de hypnose en hypnotherapie. In Pubmed vinden we met het simpele keyword ‘hypnosis’ in 2009 al meer dan 200 publicaties in peer-reviewed tijdschriften. Daarvan zijn wel een aantal niet specifiek gericht op hypnose en hypnotherapie, maar toch.

Eerst sommen we een aantal aandoeningen op, waaruit blijkt dat hypnose zinvol kan zijn. Vervolgens enkele publicaties die gewijd zijn aan zelf-hypnose. Tenslotte staan we stil nij de biologische effecten van hypnose, en hoe die te meten zijn.

Hypnose zinvol: van blaasfunctie-stoornissen tot pijn en stress bij brandwonden 

Er is een heel aantal uiteenlopende aandoeningen waarvoor aanwijzingen zijn dat hypnose zinvol is, zoals de overactieve blaas [1], postoperatieve onrust bij kinderen [2], pijn en vermoeidheid bij kanker [3], IBS [4]en IBS bij kinderen [5] en andere functionele buikklachten [6] [7], pijn en stress bij brandwonden [8], temperomanidubalaire dysfunctie [9], fibromyalgie [10], pijn en stress tijdens vaccinaties bij kinderen [11], inductie van anesthesie bij kinderen [12] na een lumbale punctie, bloedafname of een anderszins chirurgische ingreep [13] [14] [15], chronische pijn bij MS patienten [16], en bij bevallingen [17]

Ook bij kanker heeft hypnose waarde. Bijvoorbeeld tegen de vermoeidheid tijdens anti-kanker behandelingsprotocollen [18] en bij opvliegers na borstkanker- behandelingen [19], alsmede bij negatieve emoties tijdens bestralingsprotocollen. [20] Ook de ernstige bijwerking van een droge mond (xerostomie) na kankerbestralingen reageert positief op hypnotherapie [21]

Zelfhypnose ook waardevol 

Dan bestaat er ook nog een techniek die we zelf-hypnose noemen, waarbij de patient zelf leert om zich in een hypnostaat te brengen, en om tijdens die staat invloed en doorgaande invloed uit te oefenen op bijvoorbeeld depressieve gevoelens. [22] Maar ook bij ernstige somatische aandoeningen als dwarslesies, kan zelfhypnose een rol spelen. [23]

Effecten van hypnose ook meetbaar op orgaan-niveau: hersenen, hart en spieren

Ook waren er een aantal studies waarbij gekeken werd naar de invloed van hypnose op de hersenfuncties. Opvallend daarbij is dat de effecten van hypnotherapie via een aantal moderne neurofysiologische methoden goed meetbaar zijn. [24][25] Ook de invloed van hypnose op andere orgaanfuncties, zoals het functioneren van het hart, zijn aantoonbaar. [26] Ook is aangetoond dat pijnprikkels tijdens hypnotherapie de corticale velden van de hersenen niet activeren. [27] [28] Erg boeiend overigens!

Recent wees Mende erop dat de bewustzijnsstaat tijdens hypnose duidelijk te onderscheiden valt van de normale bewustzijnsstaat. De hypnotische trance heeft dus bijzondere karakteristieken. De hypnotische trance is daarom ook geschikt voor een bijzondere en meer intensieve vorm van leren. Daarvan kan dus goed gebruik gemaakt worden in de kliniek.

Hypnotic trance has special qualities as a distinctive state of awareness with the patterns of brain activities characteristic only for the hypnotic trance, setting it aside from the waking state, relaxation, sleep and even meditation. The studies performed by Ulrike Halsband (2004; 2009), Amir Raz (Raz, Shapiro, Fan and Posner, 2002), Pierre Rainville (Rainville, Hofbauer, Bushnell, Duncan and Price, 2002) and other prominent researchers produced some very clear evidence in that direction. Hypnosis allows for intensified experiential learning and can be utilized to facilitate therapeutic progress in many applications. (in: Contemp. Hypnosis 26(3): 179–184 (2009))

In het plaatje hierboven zie je de hersenen volgens een fMRI studie, waarbij pijnlijke prikkels gegeven worden (bovenste rij) en pijnlijke prikkels samen met hypnose (middelste en onderste rij). Duidelijk is dat hypnose het centrale verwerken van pijn-prikkels aantoonbaar verandert. 

Hypnose: een waardevolle techniek die veelbelovend voor de toekomst is! 

Experts die hun hele leven in de hypnose werkzaam zijn, geven aan dat er nog vrij veel onbekend is, en dat er nog veel te onderzoeken valt. [29] Om aan te geven hoe hedendaagse onderzoekers uit academische milieus tegen hypnotherapie aankijken, gaan we wat dieper in op een publicatie uit de Universiteit van Hartford (VS). [13]

Psychologen van deze universiteit publiceerden in 2009 een overzicht met als titel: The effectiveness of hypnosis for reducing procedure-related pain in children and adolescents: a comprehensive methodological review.

In dit artikel geven de auteurs aan dat de hypnose in de periode waarin het gezien werd als een soort circus -ctiviteit al lang ontgroeid is, en dat hypnose een bijzondere bijdrage kan leveren binnen de hedendaagse geneeskunde, of zoals de auteurs het zelf stellen in de inleiding van hun stuk:

Hypnosis holds great promise as a tool for alleviating the pain and discomfort experienced by children and adolescents undergoing invasive medical procedures. Once associated with stage shows and fringe therapies, hypnosis has been proven to be a very effective intervention for relieving pain in adults.  

Ze noemen hypnose dus meteen in hun inleiding al een ‘heel effectieve interventie voor de behandeling van pijn.’  En zowel acute als chronische pijnsyndromen zijn via hypnose te behandelen. .. qualitative reviews of the use of hypnosis with clinical pain have recently concluded that hypnosis is an effective treatment for alleviating both acute and chronic pain conditions..

Omdat er al zo veel aanwijzingen zijn dat hypnotherapie zinvol is bij volwassenen, richten de psychologen zich verder op de bewijzen die er zijn dat hypnose ook zinvol is bij kinderen en adolescenten. In de tweede helft van dit decennium is duidelijk geworden dat dat ook zo is:

Over the better part of the last decade, there has been a steady stream of controlled studies of the use of hypnosis for reducing procedure-related pain in children and adolescents. Reviews of this literature have generally pointed towards the utility of hypnosis.

Vervolgens bespreken ze een groot aantal studies op het onderdeel procedure-gerelateerde stress en pijn bij kinderen. Dat is dus de ellende die kinderen ervaren als ze een lumbaal punctie krijgen, een vaccinatie, beenmergonderzoek, een blaasonderzoek etc.  Opvallend is hoe eensluidend het onderzoek op dit gebied is:

With almost uniform consistency, hypnosis and hypnotic-like imagery have been shown to be more effective than no treatment, standard medical care, or attention control conditions in alleviating the discomfort associated with bone marrow aspirations (Kuttner et al. 1988; Liossi and Hatira 1999), lumbar punctures (Liossi and Hatira 2003), voiding cystourethograms (Butler et al. 2005), the Nuss procedure (Lobe 2006), and post-surgical pain (Huth et al. 2004; Lambert 1996).  

De conclusie van het onderzoek is dat hypnose bij kinderen een duidelijk bewezen nut heeft bij het verminderen van stress en pijn bij kinderen die medische ingrepen en onderzoek moeten ondergaan. Dat hypnose daarbij beter is dan de standaard behandelingen en dat hypnose dan ook vaker overwogen zou kunnen worden in de kliniek.

In sum, empirical research has demonstrated the utility of hypnosis for reducing the pain and discomfort experienced by youngsters undergoing a variety of invasive medical procedures. It is a relatively time-efficient treatment that has proven to be more effective than standard medical care or control conditions, and at least as effective as such time-honored interventions as distraction. A variety of uses of hypnosis qualify as a possibly efficacious therapy for reducing post-surgical or lumbar puncture pain. Several other hypnotic interventions have shown considerable promise relative to the criteria for empirically supported therapies as an intervention for lumbar punctures, bone marrow aspirations, venipunctures, and voiding cystourethograms. A growing emphasis on empirically supported therapies and evidence-based practice underscores the exciting potential of hypnosis as a tool for clinicians who work with children and adolescents undergoing invasive medical procedures. 

Samenvattend is door deze studies in 2009 duidelijk geworden dat we hypnotherapie niet meer kunnen opvatten als een niet bewezen alternatieve methode. Er is vanuit de reguliere onderzoekers veel belangstelling voor de waarde van hypnose bij een veelheid van aandoeningen. Ook is er duidelijk geworden dat hypnose biologisch meetbare effecten heeft in het zenuwstelsel en op orgaan-niveau.

Tenslotte citeren we graag Mende, als hij de waarde van hypnose voor de 21ste eeuw beschrijft: de integratie van hypnose in main-stream psychotherapie en geneeskunde is niet moeilijk als men in gaat zien dat het om simpele basale psychologische mechanismen gaat, en dat de effecten van hypnose ook eenvoudig te onderzoeken zijn. Dat dat zo is, blijkt al uit het vele onderzoek dat in 2009 uitgevoerd en beschreven is.

Hypnosis can be easily integrated in mainstream psychotherapy and medicine by looking at the processes underlying hypnosis: these are all basic psychological processes. Whether it is communication, attention or perception: all of these processes are researcable, explicable and very exciting. If the healthcare community and the public fi nd out more about these processes, they will know more about hypnosis – without taking away the ‘magic’ fl avour hypnosis has.  (in: Contemp.
