Hype’s doorboord!

Hype's. De nieuwe generatie middelen...veel beter dan de oude generatie... (en duurder). Iedereen gelooft de wetenschap. Gelooft de feiten. Maar de feiten liegen vaker dan we denken. De zogenaamde wetenschappelijke feiten zijn opgepompt met hete lucht. Een naald en je kan ze doorprikken! Dat werd gedaan op 21 april 2009 in een Guest Editorial in de BMJ. Laten wetenschappers van naam zien hoe wankel de fundamenten van ons medisch geloof zijn. We citeren:

Heralded as the next big breakthrough in the treatment of schizophrenia, second-generation antipsychotics were thought to offer superior efficacy and more favourable adverse-effect profiles than available treatments.

But does the evidence gathered since their arrival over 10 years ago support the early hype?

As Robin Emsley discusses in our Guest Editorial, it may be that the distinction between second-generation antipsychotics and first-generation antipsychotics is a redundant one!

Dus als we liegen, liegen we in Hoge Commissie!

Tegenstrijdig, eenzijdig en niet correct, ziedaar onze EBM 

Hoe begon dat verhaaltje eigenlijk. Met een bekende en eenvoudige manipulatie. Je doseert gewoon te hoog met de zogenaamde gouden standaard, haloperidol bij schizofrenie. Dan krijg je lekker veel bijwerkingen. En dan is jouw nieuwe stof natuurlijk veel beter.

Early clinical trials reporting advantages for second-generation antipsychotics over  typically used the high-potency first-generation antipsychotics haloperidol as the comparator drug, and at doses higher than those recommended. 

Dan heb je mooi nieuws, en dat vinden tijdschriften fijn om te publiceren!

publication bias leading to over-emphasis of positive results may have influenced opinion…

Dan vervolgens komen de meer realistische studies, en dan smelt het veronderstelde effect snel weg, aldus deze Engelse criticus:

Perhaps the strongest evidence against SGA superiority comes from three "pragmatic" trials assessing treatments in designs that more closely mimic "real world" settings, none of which found clinically meaningful advantages for second-generation antipsychotics.

Oeps! En dan de laatste uitsmijter voor de heren anti-kwakzalvers:

The evidence base for when to choose a second-generation antipsychotic over a first-generation antipsychotic, and vice versa, is inconclusive, often inconsistent, and sometimes contradictory.

This creates confusion among clinicians, and means that the "evidence" can be incorrectly interpreted, or selectively quoted to endorse an individual’s opinion.  

Kortom: we citeerden eerder niet voor niets de wetenschapstheoreticus Prof. Ludwig von Feyermorgen op deze website. Zijn visie wordt weer helemaal onderschreven door deze editorial: 

  1. Wetenschappelijke feiten zijn wankele feiten.
  2. Bewijs is veelal inconsistent, tegenstrijdig en zonder een duidelijke conclusie.
  3. Medici raken verward door deze tegenstrijdigheden door het grote geloof in waarheid van het Bewijs.
  4. Bewijs kan gemanipuleerd worden.
